A little backstory on the Search For Milli Vanilli
Search for Milli Vanilli (The)
So I think this was the first D-mod (credit to Wyndo for inventing the word btw) ever created. It was a stupid test just to see if the system worked.
It was originally released as "mill100.exe" in late 1997. The readme.txt is dated October 15th, 1997. The original file is still available here: http://www.rtsoft.com/dink/mill100.exe
In a sad coincidence, Rob Pilatus did commit suicide on April 3rd, 1998. It's important to note that this had not happened yet when I made this.
(for those that haven't played this D-Mod, in one scene is Rob is contemplating suicide but a little girl stops him by believing in him. He ultimately re-unites with Fab to perform as Milli Vanilli again in a happy ending)
The readme calls it a "Dink Addon" (the name D-mod as well as the .dmod format didn't exist yet) and requires Dink V1.01

So, did this work with the Dink Smallwood demo? Could the demo play all D-Mods?
I have a very very old memory about "The Porn D-Mod". It was whispered about on message boards and chat rooms, and I think it was only passed around from person-to-person and was never actually posted online. The author was anonymous. The rumor was that it was the very first D-Mod, but given that you (and others) have established that The Search for Milli Vanilli came out before the term 'D-Mod' was coined, that seems unlikely (or maybe it was just called "The Porn Mod"?).
I received a copy at one point (probably late 1998), and from what I recall, it was actually similar to The Search for Milli Vanilli where there were a few 8-bit photographic images that you could talk to.
I wonder if anyone actually has a copy of that somewhere.

Hmm, I don't recall any porn D-mods, but it's possible I just forgot. But yeah, no way it could have been the first one. Would be interesting to check the file dates if we could find it though.

The Dink Demo (neither the first one, or the second much larger one) allowed D-mods I believe, if the Dink demo readme is to be trusted anyway - it claims you need to buy the game for that feature to work.

SirCabbage uploaded it a few years ago and I played it. I can't figure out where it is now though. DuckLord talks about it in this thread
I never knew about Milli Vanilli until i played this dmod , they had some good songs sad how their story ended.

In fact its the only dmod i played fully on dink hd back when it supported steam overlay on steam something broke after that.
