Survey boxes from dink squad and inserting info for dink directory both use email forms. The form itself is just plain html form, but to process it that deals with server side programming (eg cgi,php, microsoft asp) If you have no idea where to start, I'll be happy to make one for you. Just email me and tell me what you want me to put down like Name, Email, Comments for example.
