The Dink Network

Hi! I thought you people might know how to help me with this... how do I make survey boxes on web pages? ones like the one on the dink squad's surveys, or when you insert your information to the dink directory and when you sumbit it it's being sent to a certain e-mail, can you please tell me how to do that?
I am working on an RPG and it'll be much easier if I'll have such an option on my site.
