The apocalypse
Why does it feature so prominently in so many good dmods (POTA, PQ, EOT...)?
Edit: by apocalypse, I mean the end of the world "as we know it", which is not the actual definition of the Apocalypse + my spelling might be wrong. But, whatever.

Scale - if you are creating an epic - it is a big story (or at least the very nature of the word epic lends it this quality)
Of course you could create an epic about a war between two countries, or a personal revenge saga that moves a character across the known (and unknown) world ... SOB was the latter...
One of PQ's core ideas was to have Dink do something that he thought was good and heroic and because of his naivety bring about the end of the world - of course once he's done that, what better story to have than he trying to fix the damage he caused.

SimonK hit the spot with that reply!
I must say nothing makes a story more "Save the World" than having an apocolypse and letting the characters fix it...or try and prevent it after it happened in the past.
Plus its a good excuse to expand the world while you revisit area's that have been laid waste.
Sincerely,
The Vault Dweller

Though it would be nice to see a bit more variation in the plots of Epic d-mods, the "End of the World" is the most obvious plotline, and is pretty compelling for the player.
But if I see one more "Dink goes to save the world... but fails! What an intelligent twist, that allows the d-mod to continue further!" plots, I'll scream (Not literally, of course, that would be going altogether too far).
Can't he just save the world first time? Ever?
