You need a normal axe as well to get the ultimate weapon. You need, both special axes, each element axe, and the normal axe, *then* the guy will make an ultimate weapon see? I never got one of the special swords either. If you unlocked the Ed the SCV secret, you can play it, and get gimme (I think, I've not done it) and gimme will allow you to get an ultimate weapon (so I heard) :)

you must have gimacs and snedfee's [weapon], [weapon]s in each element and the regular [weapon].
the guy who instilles weapons makes an ultimate [weapon] for you...
([weapon] = sword, axe or bow)
